From a medical point of view, a couple is considered to have fertility problems when they have not achieved a pregnancy after a year of unprotected sex. 15% of couples are in this situation and it is important to recognize and treat the couple as a unit.

Although infertility problems have traditionally been attributed to women in the past, today we know that in approximately half of the cases there is some type of male factor. The causes of infertility are due to the man in a third of the cases, in a third to the woman and in the other third to both (they are mixed causes because both have their fertility compromised). As the problem of male infertility is more common than originally thought, it is vital that any evaluation of the infertile couple includes a medical history of the man, a physical examination, and at least two studies of the semen (spermograms).

It is also important to note that the words infertility and impotence have a very different meaning. While the first implies the impossibility of achieving a pregnancy, the second refers to the difficulty in achieving or maintaining a sexual relationship. A man can be infertile and retain his sexual potency, or be fertile and have erectile dysfunction. Many times the man does not distinguish the difference between the two seeing his virility threatened. In this way the investigation begins and ends in the woman, to just begin to study the man when everything in her is normal and consequently delaying the consultation to the andrologist in a timely manner.
