Innovative methods of male infertility treatment in the ISIDA clinic

According to generally agreed standards, if pregnancy does not occur within a year regardless regular sex life, the couple is diagnosed with infertility and the necessity of examination and treatment arises.

There are several types of infertility: female, male, complex and also vague or idiopathic. 40% of couples have infertility due to the male factor. Male infertility is a decrease in the fertilizing capacity of spermatozoa, which occurs for various reasons: abnormalities of testicles, chromosomal abnormalities, toxic effects of polluted environment, high temperature exposure, traumas.

During the last 20 years, significant progress has been made in the field of male infertility treatment. At the moment, the treatment of patients with various forms of pathospermia is based on the implementation of assisted reproductive technologies: intrauterine insemination (IUI), in vitro fertilization (IVF) and IntraCytoplasmic Sperm Injection (ICSI).

Intrauterine insemination is performed in cases when the woman is healthy and the fallopian tubes are passable, while man’s semen characteristics after treatment are sufficient to fertilize the egg after inserting them directly into the uterus.

The development and practical application of in vitro fertilization (IVF) has become an important achievement in the field of infertility treatment. This method is a solution to the problem of infertility, which was previously considered as incurable. Now IVF is actively applied in case of problems with male fertility or after unsuccessful attempts of insemination. This method consists of several stages:

• pharmacologic stimulation of women’s follicle growth;

• aspiration of follicle contents and extraction of eggs from it;

• ovum fertilization with previously obtained and prepared spermatozoa;

• transfer of embryo to the mother's uterus after 72 hours from the moment of fertilization;

• pregnancy testing in 14 days after embryo transfer.

However, IVF method has very limited opportunities for infertile married couples, whose problem is caused by the male factor. The inability of spermatozoa to penetrate into the egg independently leads to impossibility of performance of in vitro fertilization. That is why in the ISIDA clinic in case of male infertility treatment we use the effective method of ICSI - IntraCytoplasmic Sperm Injection. It allows bypassing all barriers that prevent the living sperm, regardless of its morphology and origin, to penetrate inside the egg. High effectiveness of pregnancy is achieved using this method - 25-30%. The ICSI method allows expanding significantly the possibilities of IVF, for example, in the treatment of an extremely severe form of male infertility, when only single spermatozoa can be found in the ejaculate instead of a normal 10,000,000.

Sometimes pregnancy is possible even in cases when spermatozoa are completely absent in the male ejaculate – by getting healthy spermatozoa from the testicles or their epididymis with the use of puncture, injecting them into the egg and performing fertilization. This allows almost complete abandoning of the usage of donor sperm.
