Stage no. 1. Envisaging the Surrogacy program using the virtual communication channels. 

• Virtual consultation (via Skype/Viber/WhatsApp/Instagram/Google Duo/‎FaceTime/phone or any other video calling app that enables making the real–time video–call (or via email) with VittoriaVita International Coordinator/Case Manager/Fertility Pro. 
• Access to the VittoriaVita extensive egg donors’ database. Selection of the egg donor(s) candidate(s) profiles by the couple.
• Access to the VittoriaVita surrogate mothers’ database. Selection of the surrogate mom(s) candidate(s) profiles by the couple.
• Sending the summaries from the medical history, fertility consult (if done), ob.–gyn. consult (if done), urological consult. (if done), Diagnostic Screening reports, lab. tests’ reports, blood tests’ results, pelvic ultrasound scans’ reports, the most recent Semen Analysis (and any abnormal tests), Hysteroscopy/Laparoscopy/other gyn. surgery reports, any urological surgery reports, previous IVF/ICSI/IMSI/PICSI cycle records, miscarriage(s)/ectopic pregnancy(pregnancies)/intrauterine fetal death, etc. (via mail) to VittoriaVita (if necessary).
• Sending the Medical Certificates that transparently and inclusively provide the relevant information about the previous IVF–ICSI/IMSI/PICSI Protocol and Embryo Transfer Protocols that failed (via mail) certified, signed, and sealed by the clinic(s) /hospital(s) where the IVF–ICSI/IMSI/PICSI embryo generation event and Embryo Transfer(s) were performed (via mail) to VittoriaVita. The Embryo Transfer Certificate(s) must certify that the transferred embryos were viable, high–quality, and with high implantation potency (for Day 3 embryo: the number of cells, data on fragmentation; for Day 5 embryo: Very Early Blastocyst/Expanding Blastocyst/ Expanded Blastocyst/Hatching Blastocyst/Hatched Blastocyst, Assisted Hatching (if done), PGS report (if done), PGD report (if done)).

• Second virtual consultation with VittoriaVita medical coordinator or Case Manager concerning the chosen egg donor(s) candidate(s) and the surrogate mom(s) candidate(s) by the couple.
• Virtual matching with the egg donor(s) and the Gestational Carrier(s) (via Skype/Viber/WhatsApp/Instagram/Google Duo/‎FaceTime/ phone or any other video calling app that enables making the real–time video–call). 
• Timeframing of the first visit to VittoriaVita (notifying the medical coordinator via Skype or phone (or email) about the arrival date and time, the duration of staying in Ukraine, and the departure date and time.
• Third virtual consultation with VittoriaVita medical coordinator or Case Manager regarding the travel documents and travel questions.
• The egg donor onboarding (empowering the pre–cycle Diagnostic Screening Protocol).
• Pre–Screening and initial medical record review of the couple’s Gestational Carrier.

Sending via email the scans of the documents that should be verified and translated before the Surrogacy program is started: 

• The passports’ scans, 
• The Marriage Certificate, 
• The Medical Certificate(s), 
• The other specific documents.

The Original Documents that should be brought to the VittoriaVita office by the couple: 

• The passports, 
• The Marriage Certificate, 
• The Medical Certificate(s), 
• The other specific documents that are essential for signing the surrogacy and starting the program.

Stage no. 2. The timeframe: 1–2 days. The first arrival in Ukraine: pre–cycle Diagnostic Screens, signing the Surrogacy Contract empowerment of the Surrogacy program.

• Support with travel for the international couples (meeting at the airport, transfer/accommodations (2 nights stay is included)). 
• Coordination and management of the couple’s first visit to the Notary. Certification and notarization of the couple’s documents necessary for participation in the Surrogacy program (the passports, the marriage certificate, the medical certificate). Translation and interpretation services.
• Coordination and management of the couple’s first visit ‘roadmap’ by the Surrogacy Case Manager or Personal Coordinator. 
• The first in–person consultation with VittoriaVita Fertility Pro (the couple’s medical history review, pelvic ultrasound scan of uterine and pelvic physiology and pathology, Antral Follicle Count (AFC), the partner’s Sperm Collection for Semen Analysis, the other Diagnostic or/and Screening tests that should be and can be initiated during the first consultation), Surrogacy pack selection, and generation of the IVF Protocol.
• Real–time matching with the egg donor and the Gestational Carrier. 
• Referrals to the experienced lawyer(s) in the field of Surrogacy for drafting the legal contracts between parties (the couple and the Gestational Carrier). Discussion of the terms and conditions that can be attached to or detached from the contract’s content. Issuing the final legal Surrogacy contract that will be signed by the parties.
• The signing of the legal Surrogacy Contract with VittoriaVita Surrogacy Agency. Notarization of the Surrogacy Contract. 
• Filling in and signing the forms, completing and signing the other medical documents necessary for the program.
• Criminal background checks envisaged for the intended parents, and for the surrogate mom.
• Psychological Screening Protocol empowered for the couple, for the surrogate mom.
• Pre–Surrogacy Diagnostic Screening of the couple (the Diagnostic Screenings, lab. tests, ultrasound scans, and other essential screenings). 
• Consultation with the Fertility Pro concerning the Diagnostic Screening results. Prep. and empowerment of the donor’s medicated Controlled Ovarian Stimulation (COS) Protocol using the hormones (Follicle–Stimulating Hormone (FSH)/Luteinizing hormone (LH).

Stage no. 3. Post–departure remote mode of the Surrogacy program: Egg Donation, Fertilization, Embryo Transfer, early pregnancy tests, early pregnancy monitoring, getting from the Case Manager the translated medical record reviews, getting in touch with the Personal Case Manager, and the surrogate mother. 

• The pre–cycle Diagnostic Screening Protocol for the egg donor, and for the Gestational Carrier including the consultations, Screenings, Diagnostic tests, lab. tests, etc.
• Prep. and empowerment of the surrogate mom’s medicated endometrial prep. Protocol using the hormones.
• Prep. and empowerment of the Controlled Ovarian Stimulation (COS) Protocol for the egg donor.
• Management and monitoring of the surrogate mom’s medicated endometrial prep. Protocol (ultrasound scans, blood tests, lab. screening tests, other tests if necessary).
• Management and monitoring of the donor’s hormonal COS Protocol (ultrasound scans, blood tests, and lab. screening tests).
• Prep. of the donor to the oocyte pick–up (pre–Oocyte Retrieval consultation(s), Screening(s)/Diagnostic tests, selection of the anesthetic). 
• Ultrasound–guided Oocyte Retrieval. Collection and their prep. of the retrieved fresh oocytes for fertilization.
• ICSI, IMSI, PICSI Embryo Generation using the intended father’s fresh sperm and the donor’s fresh oocytes. 
• Prolonged Embryo Cultivation until the blastocyst stage including embryo development monitoring in EmbryoScope. 
• Empowerment of the PGD–5, PGD–9, or PGD–24 Protocol enabling the comprehensive Screening and Gender Selection. 
• Mock Embryo Transfer (ultrasound–guided mock Embryo Transfer) if necessary. 
• Prep. of the surrogate mom for the Embryo Transfer (consultations, ultrasound scans, blood tests, the other diagnostics if necessary).
• Embryo Transfer (unlimited transfer trials are included), and the compensation paid to the surrogate mom for the transfer (unlimited).
• Vitrification of remaining embryos for the future FET cycle(s) and fee–free storage of the cryovials during 12 mo. in the cryobank.

Early pregnancy tests’ timeline: 

• Urine Pregnancy Test is done on day 11 after Embryo Transfer; 
• HCG blood test (Human Chorionic Gonadotropin) is done on day 14 after the Embryo Transfer. 

Early pregnancy ultrasound scan on day 28 after transfer to confirm the viable or non–viable pregnancy. 

If Embryo Transfer didn’t result in a viable pregnancy, the new FET or ICSI, IMSI, PICSI Embryo Generation cycle is initiated fee–free.

Getting from the Case Manager the translated medical record reviews: the consultations’ summaries, the Diagnostic Screenings and tests results, the COS Protocol, the fertilization Protocol, the Embryo Transfer Protocol, the early pregnancy tests’ results, and the other medical record reviews.
