1. ‘Who offers IVF–ICSI with the donor’s oocytes and the partner’s sperm with PGD Protocol empowered for the Diagnostic Screening and the baby’s Gender Selection (or without PGD Protocol)?’

Payment Schedule due upon the following process milestones. 

Every Treatment pack has its own financial ‘roadmap’ with the payment timeframe. Payment methods: Bank Drafts, Wire Transfer, Cash, Visa /MasterCard. All prices are subject to change. 

VittoriaVita Surrogacy agency costs are paid in multiple installments:

• First Installment: 3,600.00 EUR (€) paid for the initial in–office meeting and IVF Contract Signing.
• Second Installment: 3,000.00 EUR (€)) paid before the Oocyte Retrieval.
• Third Installment: 1,000.00 EUR (€)) paid before TESA (if TESA is necessary).

If TESA–empowered Sperm Retrieval pack is ‘integrated’ with the core IVF pack, the TESA fee covers monthly Sperm Retrieval and is charged before TESA or can be paid when signing the IVF contract.

The IVF–ICSI with the donor’s oocytes and the partner’s sperm program’s total cost starts at 6,600.00 EUR.

2. The ‘core’ extras that can be attached to IVF–ICSI with Egg Donation and the partner’s sperm, and their fees: 

PGD–5 (Preimplantation Genetic Diagnosis for 5 chromosomes) pack’s cost starts at 1,000.00 EUR. This fee covers screening of up to 12 embryos. For each extra embryo, the envisaged PGD–fee is 90.00 EUR (€). 

PGD–9 (Preimplantation Genetic Diagnosis for 9 chromosomes) pack’s cost starts at 1,500.00 EUR. This fee covers screening of up to 12 embryos. For each extra embryo, the envisaged PGD–fee is 90.00 EUR (€). 

PGD–24 (Preimplantation Genetic Diagnosis for 24 chromosomes) pack’s cost starts at 2,500.00 EUR. This fee covers screening of up to 10 embryos. For each extra embryo, the envisaged PGD–fee is 250.00 EUR (€). 

Embryo Vitrification and Storage of the vitrified embryos (embryonic cohort) in the cryobank during 12 months: 

• Embryonic cohort prep. for Vitrification + 
• Vitrification + 
• Storage of the vitrified cryovials in the cryobank pack’s cost starts at 400.00 EUR. One extra year of vitrified embryos’ storage costs 300.00 EUR per cycle.

Extra Frozen Embryo Transfer cycle ‘installed’ in the current IVF pack’s cost starts at 1,650.00 EUR (€). 

Extra IVF–ICSI with the donor’s oocytes and the partner’s sperm (if there are no embryos left for empowering the transfer Protocol) without PGD Screening pack’s cost starts at 4,850.00 EUR (€). 

TESA–Sperm Retrieval pack ‘PRO’: 

• ultimate surgical Sperm Retrieval pack: preoperative medical examination + 
• the partner’s surgical Sperm Retrieval (TESA Protocol) with Sperm Collection and prep. for being used in IVF–ICSI cycle (1,000.00 EUR per mo.).

Sperm Freezing (Cryopreservation or Vitrification technique) and Storage of the frozen or vitrified sperm samples during 12 months’ (for the future empowerment of the IVF–ICSI (IMSI, PICSI) Embryo Generation Protocol using own or the donor’s oocytes and the devitrified partner’s sperm): 

• Sperm Collection and prep. for Vitrification +

Pack’s cost starts at 450.00 EUR per year. 

One extra year of cryopreserved or vitrified sperm samples’ storage costs 400.00 EUR (the payment is done before several business days the previous payment will be expired). The pack’s fee transparency: Cryopreservation or Vitrification of sperm samples costs 50.00 EUR plus storage of the cryovials with vitrified sperm in the cryobank costs 200 EUR per 6 months or 400.00 EUR per 12 months. 

The other out–Bundle fees that are charged (unique circumstances): 

• Canceled cycle fee for restarting the program.
• Replacement of the egg donor on the couple’s demand after the COS Protocol for the previous donor has been started.

• Management and monitoring of the donor’s hormonal COS Protocol (blood tests and lab. screening tests) +
