Age limitations: the age of 50 years at the latest is the upper age limit for undergoing Embryo Transfer at GYN — FIV a.s. clinic. 

Payment Schedule due upon the following process milestones: 

100% — upon signing the contract (payment should be done before each Treatment cycle is started). Payment methods: Bank Drafts, Wire Transfer, Cash, Visa /MasterCard. All prices are subject to change.

3. ‘What is not in the Bundle?’ What extras can be attached if necessary, to the IVF Protocol? 

The basic pack’s fee does not cover the pre–cycle Diagnostic Screening, serology tests, Andrology Screening fees, surgical Sperm Retrieval, the medication necessary for empowering the endometrial preparation Protocol, the medication necessary for empowering the spermatogenesis stimulation Protocol (if needed), and the other extras that are not ‘attached’ to the basic Protocol. 

Consultation fees. Consultations in a foreign language for patients not being insured in Slovakia:

• Consultation in a foreign language (primary health care, ultrasound examination), 30 min. — 35.00 EUR. 
• Consultation in a foreign language (one–day surgery, Infertility Treatment), 30 min. — 70.00 EUR. 

Diagnostic Tests:

• Complex initial laboratory examinations (before ‘booking’ the IVF pack) — 250.00 EUR. 
• Serology Tests (Sexually Transmitted Diseases: HIV 1+2, Syphilis, Hepatitis B+C, blood type + RH factor) — 50.00 EUR.
• AMH test (Anti–Müllerian Hormone Test) — 40.00 EUR. 

Medication:

Medication used for empowering the Controlled Ovarian Stimulation Protocol (the Hormonal Stimulation) costs approximately 1,000.00 EUR.
