First transfer — made upon signing of contract — 30 000 USD

●Medication for the surrogate mother for preparing the endometrium, getting her ready for the embryo transfer, giving medication until the heartbeat confirmation of pregnancy is achieved;

●Agency fee for providing the surrogate mother for every embryo transfer attempt after signing the agreement, surrogate mother can be maintained or changed for every attempt. Second transfer — after pregnancy confirmation — 13 600 USD

●Surrogate mother gets 400 USD per month during nine months and that totals to — 3 600 USD.

●Looking after the surrogate mother during the pregnancy. Setting the doctor’s appointments, giving medication and vitamin supplements, arranging ultrasound scans, communicating with the intended parents’ daily bases and unlimitedly covers hospitalization expenses of the surrogate mother — 4 000 USD.

●Part of the Guaranteed program fee that is covered by the second transfer fee — 6000 USD.

Third transfer — on week 28 of pregnancy — 1 500 USD

●Maternity hospital expenses for the natural delivery. This fee covers the private doctor’s fee, the surrogate mother and the newborn care for 4–5 days in the hospital — 1500 USD.

Fourth transfer — after the baby’s delivery — 12 400 USD

Notes:

●Accommodation for the intended parents (IP) while waiting for the baby registration is included. Accommodation covers maximum 3 months’ period. Budget for accommodation is 1000 USD for entire staying. In case parents are willing to arrange the accommodation themselves, New Life is happy to refund 1 000 USD. Utility bills are not included and parents are paying upon staying.

●We arrange the airport pick–up and drop off upon your arrival and leaving.

●One–month babysitting is included with 8–hour nanny shift.

In the Framework of Guaranteed Surrogacy with egg donation package we offer:

●Unlimited IVF/ICSI procedures;

●Unlimited coverage of involved medical services fees;

●Unlimited number of new egg donors wherever required;

●Intended Parents willing to use their own genetic materials for IVF are welcome to start guaranteed program with us with their own eggs. However, Intended Mothers AMH should be above 1 and she should have at least 7–8 antral follicles. If after first IVF—ICSI with embryo transfer no pregnancy is achieved, and parents have no frozen embryos to use up, they are supposed to move to Egg Donor cycle directly. Parents need to be ready to transfer to Egg Donation cycle.

●Intended Parents have to agree to leave frozen sperm in the clinic which will be sufficient for 5 or more attempts. In case more attempts are required and no sperm is available, intended parent is obliged to travel to Georgia again for collection of the genetic material (sperm collection with further preparation of the sperm sample for ICSI).

●Intended parents are allowed to choose the desired egg donor themselves through New Life Egg donor database. (Only local Georgian egg donors are available for this surrogacy cycle.)

●After having successful delivery, the intended parents are obliged to notify us about the fate of remaining frozen embryos: either embryos will be kept for the next frozen embryo transfer cycle or discarded upon intended parents’ request or given for the donation after intended parents’ approval.

●Intended parents are obliged to cover payments of the remaining frozen embryos after one–year storage themselves.

●In case of twin pregnancy, additional 2 000 USD is payable to the surrogate mother. (Additional 2000 USD is only payable if babies are born from 34 to 40 weeks, if delivery happens earlier contract payment Annex terms apply.

●In case of the surrogate mother’s miscarriage, surrogate mother’s compensation at certain stage of pregnancy is payable under the terms of the surrogacy contract payment annex.

In the framework of Guaranteed Surrogacy/Egg donation package we do not undertake following responsibilities. 

●Coverage of the costs of premature delivery neither before 28 week of pregnancy nor after 28 weeks.   (Parents can purchase premature delivery insurance at a cost of 5000 USD).

●Amniocentesis cost (1000 USD). Amniocentesis will only be done in case it is medically indicated and recommended by the genetics’ specialist.

●In case of no immediate success (the pregnancy is not achieved after the first IVF—ICSI cycle completed and the embryo is transferred to the Gestational Carrier’s uterus, but the pregnancy is not viable), New Life will not make any refund and reserve its rights to continue attempts until successful delivery of the baby. 

●Embassy related expenses.

●NICU Care (preterm newborn should stay in newborn intensive care unit), the cost is 350 USD per night in Georgia. Full Coverage of Premature baby delivery insurance can be purchased at a cost of 5000 USD per baby. NICU Care Insurance should be purchased before 12 weeks of pregnancy.

●Surrogate Mother’s payment insurance can be purchased at a cost of 2500 USD. This insurance will cover the surrogate mother’s payment in case if baby dies after premature delivery from 20th week to 40th week. This Insurance will also cover the abortion fees in case baby is not genetically healthy and must be aborted. Surrogate Insurance package cost should be paid until 12 weeks of pregnancy.

●In case if baby survives, parents (and not insurance) are obliged to cover the surrogate mother’s payment according to the contract.

Important Notes:

●●For the Guaranteed Surrogacy with egg donation packages we do transfer 2 embryos for each trial.

●●In case twin pregnancy is achieved, we do not do embryo reduction.

●●In case New Life Partner IVF clinic or the clinic owned by New Life fails and clinic’s success rate is decreased, New Life remains the rights to transfer the frozen sperm to another clinic and perform the guaranteed program in another clinic.

●● In case of twin pregnancy additional 2 000 USD is payable to the surrogate mother. (Additional 2000 USD is only payable if babies are born from 34 to 40 weeks, if delivery happens earlier contract payment Annex terms apply.

●●If the twins are born and one of them survives and other dies, the parents shall pay the surrogate’s compensation that is due for twin delivery fully. In such case the program shall be considered completed and New Life’s commitment shall be considered as fulfilled.

●●PGS for 24 Chromosomes covering screening up to 8 blastocysts (up to 8 embryos) — 3 500 USD. In case more blastocysts are received and biopsied, an additional fee 350 USD is paid for each additional embryo screened. Payment is applicable per each fresh IVF cycle.
