Oocyte uptake

When the size of the follicles is adequate, a hormone is administered that will produce a controlled ovulation. It is administered 36 hours before oocyte uptake. This collection is performed in the operating room under sedation, using a special needle, which through the vagina, collects the fluid found in the follicles, and which in turn contain the ovules. In the laboratory is where they will be separated from this fluid and placed in culture medium in an incubator. After one or two hours, the patient can leave the hospital.

Fertilization

Once we have the sperm sample, it is processed to inseminate the ovules. In a culture plate several microdrops are prepared, in which we will place an adequate concentration of sperm. Later, we place an egg in each of these microdrops, and in this way, in a few hours, one of the sperm will enter the egg, producing fertilization or fertilization. (This is properly in vitro fertilization.) This plate with the ovules and sperm is left in the incubator, thus being in conditions similar to the physiological ones, that is, 37º C, with a concentration of 6% of CO2 and high humidity relative (95%). At 18-20 hours after insemination, we verify if fertilization has taken place, observing them under the microscope.

Embryonic culture

The embryos obtained (which are the previously fertilized ovules) are going to spend one or two more days in the incubator, observing their evolution, to determine their quality and thus be able to select the best quality embryos to later transfer them.

Embryo transfer

After these days in the incubator is carried out the transfer of one, two or three embryos, depending on the number of each case in particular, according to medical advice and desire of the patients. The total process takes no more than half an hour. The embryos are introduced into the uterine cavity through a very thin cannula, guided by ultrasound, to place the embryos in the most appropriate place. It is expected then that implantation occurs that could lead to pregnancy.

I’m now 30 weeks pregnant after my 1st cycle awaiting my ladybundle! A huge Thank you to Dr. Fernando Hernantez and all Irema team for turning my dream into a real thing! Those tiny booties love to kick me from inside, I feel them... And I am so thrilled as every week those tiny booties become plus size (soon there will be two pointe shoes inside)! Thank you, dear team!

I asked what tests I should undergo and is there something…I can just, well, skip. Dr. Fernando Hernantez explained everything what is essential in testing and what is not so crucial. After I got the results, I spoke to him about the medications and the side effects as I had Quincke’s edema three years ago so I could be allergic to the meds. He scheduled for me two blood tests to investigate what will be if I start these meds. We crossed out those medications that could lead to Quincke’s edema and started the mild stimulation. After the retrieval, I asked him about the most perfect techniques to make my tiny bundle feeling loved and cared. He proposed assisted hatching to help my baby to hatch outside and increase the implantation chances. And mock ET (ET is “embryo transfer”) to get navigated through the embryo launching prior to actual embryo transfer. And we did it!

Here, at Irema everything was different. From the first moment I was here, everything was so amazing! I was cordially greeted by Frederica at the Reception (she is very, very, very sweet). After that, I was warmly met by Dr. Javier Martinez. You know, sometimes shining eyes, warm smile and cordial conversation is everything. Literally everything. We were talking and talking about all treatment options, after that I was totally amazed when Dr. Javier Martinez told me that we can “look around” and make a small excursion around the clinic. He showed me the embryolab, we greeted together Dr. Fernando Hernantez and I caught a glimpse of first embryo debut! Surely, I didn’t enter the lab but I stood with Dr. Fernando Hernantez and he explained to me everything about embryo creation event. I felt that everything he did for me was so welcoming. I never thought that I could find the person who would be so sympathetic and so perfect listener.

I was so anxious every time I entered the new fertility clinic. In three clinics I faced such a coldness. No sympathy, just: “Let’s start with scheduling tests for Wednesday and don’t forget to pay fees today or tomorrow!” “Yes, we can start the IVF cycle in two weeks. Medications are standard. We don’t have minimum stimulation protocols but be sure, the protocols I propose you are trusted and perfect for you. Tests should be done, and blood work every time you are here as we need to manage the situation. What? Sensitive vessels? There will be too many syringes, and your delicate vessels, well, try to cope with your fair and bruises.”

In one clinic I entered the office of the RE and stayed in the doorstep for ten minutes waiting for her to finish the conversation with her lower. She glanced at me three times, and I saw such contempt in those huge dark eyes. After that, she irritably said: “Hello. Five more minutes, I want to drink my cappuccino. I will call you, and close the door?”

Dear IREMA Team, thank you! I was so nervous when choosing a fertility clinic. No, not just nervous, I was a real bundle of nerves. Everything was so confusing at the very beginning. I was googling and googling the information about the treatments, pre–treatment tests, techniques they are using, fertility medications, prenatal vitamins, and many, many other things.

I was really overwhelmed to know that the IVF cycles are different. There are mini–IVF cycles with minimal ovarian stimulation protocols, there are IVF cycles that have flexible treatment protocols, needle–free IVF cycles for the needlefobes, IVF cycles for the extreme cases when the stimulation is very intensive and the IVF cycles that can be started, after that canceled and started again. I designed my “Clinics’ list.” I listed there all clinics with the treatments they offer and called to schedule the consultations with RE (by the way, RE is “Reproductive Endocrinologist abbreviation)
