In FIV Marbella we offer standard egg freezing cycles that can be synchronized with oocyte thawing procedure, conventional in vitro fertilization (the simple technique that involves placing the oocytes and the sperm into the same Petri–dish and 24 hours of waiting for fertilization to occur). The alternative for conventional in vitro fertilization is ICSI fertilization technique. Intracytoplasmic sperm injection fertilization technique (more advanced technique that involves placing every oocyte under the powerful microscope’s objective for micromanipulation, fixing it with oocyte holding pipette (the oocyte holder micropipette), immobilizing the single spermatozoon by catching its tail with ICSI micropipette for further spermatozoon’s transfer directly into the oocyte’s cytoplasm, the microinjection of the oocyte with ICSI micropipette and transferring the single spermatozoon inside the oocyte, transferring the fertilized oocyte to the incubator and 24 hours of waiting for fertilization to be completed (the chromosomal content to be blended and bounded). Or IMSI “Intracytoplasmic morphologically selected sperm injection” fertilization technique will be used. IMSI technique’s major difference is SELECTION of the top–quality spermatozoa prior to microinjection. The other stages of fertilization are the same as for the ICSI technique. And the last stage of IVF cycle — the embryo transfer.

STEP 1 | INITIAL CONSULTATION with FIV Marbella Fertility Expert (or with FIV Marbella Fertility Experts, in case if necessary)

During initial consultation, the fertility expert will draft your egg freezing cycle (ovarian stimulation protocol, the list of meds for controlled ovarian stimulation, pre–schedule the ultrasound scans’ approximate time frame (the ultrasound scans are done for visual management of your follicle growing, pre–schedule the blood tests that will be done to measure your hormonal levels and for “indication” of the follicle growing, and scheduling your pre–treatment screening tests). Every step will be discussed with you.

STEP 2 | Preparation for egg freezing cycle

Pre–treatment screening tests and blood tests are performed for making the “tuning up” of the drafted egg freezing protocol according to the obtained results. The medications (fertility drugs) for ovarian stimulation and injectable hormones are prescribed. The nurse will teach you how to do self–injections into the tummy’s zone and you will have the first self–injection is done under the nurse’s “management.”

STEP 3 | CONTROLLED OVARIAN STIMULATION and ITS MONITORING

Egg freezing cycle involves a controlled ovarian stimulation protocol that lasts for 12 days. Ovarian stimulation will be closely monitored via ultrasound scans and blood tests. This “dual–control system” makes it possible for your fertility expert to monitor ovarian stimulation cycle and to manage almost issues.

Analytical control of the hormonal levels, analytical control of the hormonal balance, and analytical control of estradiol hormone indicates the oocytes’ maturity. Alternatively, ultrasound scans “visualize” the follicle maturation and the timeline for triggering the ovulation (for scheduling the TRIGGER SHOT — an injection done to prevent spontaneous ovulation (losing the mature oocytes).

STEP 4 | TRIGGER SHOT

The trigger shot is injected to prevent self–ovulation and making oocytes’ collection accessible. After the trigger shot is done, there will be a short timeline to get ready for the ultrasound–guided oocyte retrieval procedure.

STEP 5 | OOCYTE RETRIEVAL

The egg retrieval procedure is an invasive one, therefore, it is performed in an operating theatre. Prior to oocyte pick–up, the patient will be sedated. The oocyte pick–up surgery usually lasts 10–15 minutes. This procedure is performed under ultrasound guidance: a fine needle “punctures” the follicles and the oocytes with liquid are collected from the follicles and transferred to the laboratory for further investigation and fertilization. Ideally, 8–10 mature oocytes will be retrieved. After that, the patient is discharged, and the recovery timeline takes 24–48 hours.

STEP 6 | OOCYTE VITRIFICATION

All retrieved oocytes are transferred into the cryovials, cryo–protectors are added to prevent the oocytes’ damage and the oocytes’ DNA content damage and oocyte vitrification protocol’ “mode” is “turned on.” Oocyte vitrification is the ultra–rapid cooling technique that involves preparing the oocytes for cooling, adding cryo–protectors, transferring the cryovials with the oocytes and cryo–protectors into the vitrification system, and vitrifying them. Vitrification is cooling the oocytes in the shortest timeframe. Oocytes are cooled literally “at a glance.” This cryopreservation technique has dramatic survival chances after the thawing procedure.
